The Hong Fook Community-Based Adult Services (CBAS) Centers provide a supportive environment for frail seniors and adults with disabilities. Our team of healthcare professionals works to help participants regain their independence while offering much-needed relief to family caregivers.

Eligibility

We serve medically eligible frail adults, with no charge for Medi-Cal participants and a sliding fee scale for others. 

Family Bridges, Inc., as a Federal Transit Administration (FTA) fund recipient ensures that both of our Hong Fook Adult Day Health Care and Hong Fook – Harisson Street Centers are in compliance with the Department of Transportation (DOT) Title VI Regulations of the Civil Rights Act of 1964. We assure that our Hong Fook Centers’ participants receive quality transportation services, which provides that no person shall, on the grounds of race, color, or national origin, be excluded from participation in, be denied the benefits of, or l be subjected to discrimination under any program or activity that receives Federal financial assistance.
